Position Overview

St. Charles Medical Group is seeking a Family Medicine Advanced Practice Provider (NP/PA-C) to join St. Charles Family Care at the La Pine Family Care Clinic. You will provide outpatient care to patients of all ages, including evaluation, diagnosis, treatment, and documentation of each encounter, under the general supervision of the Clinical Division Director and direct supervision of a designated physician.

This role is built around a Medical Home model. You will be supported by in-team Behavioral Health, Pharmacy, Care Managers, Community Health Educators, and administrative staff, with a Family Care physician providing back-up and expertise as needed. The goal is evidence-based, personalized care tailored to each patient’s individual needs.


Work Schedule & Call Expectations

  • Minimum of 40 hours per week, including a minimum of 36 hours of patient contact time
  • Regular, exempt, first-shift position; scheduled days may vary with clinic operating days and hours
  • Participation in the clinic call rotation, shared equally by all providers in the call group, including evenings, weekends, and holidays
  • Family Care physician back-up and support available as needed

Key Responsibilities

  • Practice evidence-based Family Medicine across diagnosis, definitive and supportive treatment, follow-up, and preventive care
  • Create treatment plans that incorporate Behavioral Health, RN Care Management, therapy, pharmacy, nutrition, or surgery
  • Coordinate medical, ancillary, nursing, and other services to achieve the best and most cost-effective outcomes
  • Monitor ongoing care plans and refer to specialists based on patient progress and condition
  • Consult with the Care Team Pharmacist on poly-pharmacy patients and the Care Team Psychiatrist on psychotropic prescribing
  • Interpret and report on diagnostic tests within 72 hours for urgent and 7 days for routine results
  • Educate patients and families on preventive care, medical issues, and prescribed treatments
  • Accurately document, code, and bill for all care in the EHR in a timely manner
  • Support the delivery, measurement, and improvement of quality health measures

Qualifications

Education

  • Master of Science in Physician Assistant Studies or in Nursing (required)
  • Doctorate of Science in Physician Assistant Studies or Doctorate of Nursing Practice preferred

Licensure & Certification

  • Unrestricted PA and/or NP license; Oregon licensure preferred
  • PA: NCCPA certification and ability to establish and maintain a Practice Agreement with a Supervising Physician
  • NP: AANC or ANNP certification with a population focus appropriate to Family Medicine (e.g., FNP)
  • Unrestricted Federal DEA number; DEA registered in Oregon preferred
  • Unrestricted ability to participate in Medicare, Medicaid, and other government health care programs
  • Current AHA BLS; AHA ACLS also required if practicing moderate or deep sedation

Experience

  • Experience providing outpatient Family Medicine care (required)
  • Experience using EHR software (required); Epic experience preferred
  • Experience working in a team environment and building consensus among health care professionals (required)
  • 2+ years in an active Family Medicine practice preferred
  • Familiarity with the Medical Home care model preferred
  • Experience providing, documenting, and billing Annual Medicare Wellness Visits preferred

Skills & Competencies

  • Advanced written and verbal communication skills with a diverse patient population
  • Knowledge of quality measures, data collection, and outcome review and improvement
  • Strong collaboration with Family Medicine, specialty care, and referring provider teams
