Position Highlights
- Position: Advanced Practice Provider (APN/PA) – Center for Cardiac Health - Heart Failure
- Location: Edward Hospital, Naperville, IL
- Full Time/Part Time: Full-Time (average of 40 hours/week)
- Hours: Monday – Friday between 7:00AM – 5:30PM. Call in the future may be required.
- Required Travel: Possible travel to Elmhurst Specialty Clinic may be required.

Job Description
Exciting opportunity to Join Endeavor Health’s Center for Cardiac Health–Heart Failure team in an autonomous role delivering patient-centered care to high-acuity patients under the direction of a collaborating physician! Key responsibilities include:
- Obtaining comprehensive patient histories and performing physical examinations
- Ordering and interpreting diagnostic tests
- Developing differential diagnoses and prescribing evidence-based pharmacological and nonpharmacological treatments
- Managing acute and chronic conditions
- Providing ongoing education to patients and their families
- Coordinating care with physicians and interdisciplinary teams to optimize outcomes and support post-discharge follow-up and transitional care
- Supporting quality-improvement and patient-safety initiatives
- Contributing to the development and implementation of organizational and service-line goals
- Adhering to all Endeavor Health policies, procedures, and standards.
Qualifications
- License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
- Education: Completion of master’s level program in specialty area
- Certification: BLS/CPR certification required
- Experience: Prior APN/PA Cardiology experience required
Special Knowledge and Skills Required
- APP experience in cardiology required
Preferred Qualifications
- Experience working with Heart Failure patients preferred
