The Cadence Medical Group of MDs, NPs, RNs, LPNs, MAs and certified health coaches supports patients between visits, reinforcing provider guidance and keeping care plans on track. Cadence's nurse practitioners lead remote clinical decision-making for heart failure, hypertension and type 2 diabetes, including guideline-directed medication titration, in close collaboration with patients' own physicians and the Cadence care team. Results include a 230% increase in heart failure patients on guideline-directed medical therapy and a 27% reduction in hospital admissions.
